Case Management

Bay County Department on Aging Case Management Team

Case Managers complete in home visits to evaluate those Bay County Residents 60 years and better for services provided by Department on Aging, such as Home Delivered Meals, Homemaking, Personal Care, and referrals to other agencies and programs as needed. The Case Management team is an advocate for resources.
Each Case Manager covers a specific area of Bay County.

An intake will be completed and a Case Manager will be assigned, with an in home visit to follow.
